In practice, NHS audiology waits for ear wax in most Integrated Care Boards now run 12–24+ months. Many GP surgeries removed ear-care equipment entirely; the nurses who used to do syringing were retrained or retired without replacement.

The procedure didn't get less safe, less effective, or less needed — it just moved out of NHS primary care. If you can wait 12–24 months for NHS audiology, that's still an option (ask your GP for a referral). For most patients with symptomatic wax, paying privately for same-week microsuction is the practical route.

Why is wax building up?

Wax is produced by glands in the outer ear canal as part of normal ear hygiene. It self-cleans for most people most of the time. It builds up when the natural outward migration is disrupted by:

  • Cotton buds (the single biggest cause)
  • Hearing aids
  • In-ear headphones / earbuds
  • Narrow or hairy ear canals (genetic, more common with age)
  • Increased wax production (often older adults)
  • Previous ear surgery or skin conditions

When should I book microsuction?

When it's causing symptoms: dulled hearing, pressure or fullness, mild discomfort or itching, new tinnitus, hearing aids whistling. Ear pain with fever, discharge or sudden hearing loss is not just wax — those need clinical assessment first.

What happens at the appointment?

20 minutes total (30 first visit). Brief intake. Otoscopic exam with video screen so you can see what we see. Microsuction under the binocular microscope, 5–10 minutes per ear. Post-procedure check. The main sensation is the loud suction noise close to the eardrum (like a vacuum cleaner held near your ear). Clinician works slowly, stops if you feel pain.

Microsuction safety

Microsuction is the safest professional method for ear wax removal. No water means no risk of pushing wax further in, no infection risk from contaminated water, suitable for perforated eardrums (with extra caution) and post-surgery patients. Water-based methods (syringing, irrigation) are 'blind' and carry these risks.

Hearing-aid users

Routine 6-monthly microsuction is sensible. The aid contributes to wax buildup, accumulated wax reduces aid performance and can damage the receiver. Bring your aids; we'll check fit after wax removal.

Aftercare and prevention

Keep water out of your ears for 24 hours. Hearing should be immediately clearer. Prevention: never cotton buds; 2–3 drops olive oil weekly if wax-prone; routine 6–12 monthly cleaning if you're a hearing aid or in-ear headphone user.
